$The Fujin Raijin II roller coaster tragedy occurred on May 5, 2007, at Expoland, an amusement park in Suita, Osaka, Japan. This incident led to the death of Yoshino Kogawara, a 19-year-old university student, and injured 19 other passengers. At approximately 12:50 p.m. on Children’s Day, a national holiday in Japan, the Fujin Raijin II, a stand-up steel roller coaster manufactured by TOGO, experienced a catastrophic failure. The second car of the six-car train derailed after a critical axle fractured, causing the car to tilt sharply and strike a guardrail along the emergency walkway. Kogawara was fatally injured when her head collided with the handrail. The coaster continued for about 300 meters before coming to a stop. Among the injured, two were reported to be in serious condition. An investigation revealed that the axle responsible for the derailment had not been replaced since the coaster’s installation in March 1992, despite being due for replacement in February 2007. The park cited insufficient workspace as the reason for not conducting the necessary disassembly and inspection. Additionally, similar cracks were discovered on a second train during subsequent inspections, indicating systemic maintenance failures. In July 2008, three former Expoland employees admitted to professional negligence. They acknowledged deliberately postponing mandatory inspections and submitting false safety reports, even after visible cracks were detected on the axle months prior to the accident. Following the incident, Expoland temporarily closed to conduct comprehensive safety checks. The park reopened in August 2007, but the public’s trust had been severely damaged. Visitor numbers plummeted, and the park struggled to secure sponsorships. Ultimately, Expoland ceased operations in February 2009, burdened with significant debt. The Fujin Raijin II accident prompted nationwide scrutiny of amusement park safety standards in Japan. Similar roller coasters were voluntarily shut down for inspections, leading to the discovery of comparable maintenance issues.
